EVENT PAPERWORK

Annual Liability Release Form ... This form needs to be filled out (front and back) in order for a student to attend an event with us. Once this form is filled out it will remain on file for a year. Every June we start over with these forms and require new ones.

Specific Event Waiver (Half Sheet Permission Slip) ... This form needs to be filled out for every event unless otherwise indicated on our calendar. This form helps prevent us from having parents fill out or Annal Liability Release Form for every event.

Scholarship Application ... We occasionally provide scholarships for camps and Youth Convention. Read event brochures for these events for all the details.
